Job Summary

The Safe Harbor Liaison will educate the youth and family regarding the Safe Harbor Docket process, will identify the youth's needs based on their assessments, will develop a plan for treatment with community agencies, and will provide case management services while the youth is assigned to the docket.

 

Essential Duties and Responsibilities

Provides care coordination/case management functions that support evidence-based practices for pre and post-adjudicated youth and their families in order to reduce recidivism and to minimize involvement with the Cuyahoga County Court of Common Pleas Juvenile Division system.

Develops individualized treatment plans for participants on the Safe Harbor Docket and makes referrals to collateral agencies.

Attends all Safe Harbor Treatment Team meetings and court hearings.  Provides written reports on each case, makes an oral presentation at the hearings and meetings, provides recommendations to jurist, and files motions as necessary.  Prepares Ohio Youth Assessment System (OYAS), completes placement referrals when necessary, and maintains case notes.

Attends various court hearings for youth being considered for Safe Harbor or in need of a Human Trafficking Victim assessment and promptly facilitate referrals.

Provides ongoing case management services to Safe Harbor participants including mediations, crisis intervention, monitoring behavior, developing treatment plans, re-evaluating treatment plans and modifying plans as needed.

Interviews, assesses, and counsels youth and families in their home, school, detention center, probation office or elsewhere to obtain relevant information for court presentations and/or further service planning.

Maintains contact with youth, parent/guardian, schools, counselors, team members.  Collaborates with service providers regarding case developments and coordinates team meetings.

Collects statistics on all Docket participants, maintains updated Safe Harbor Tracker document, and submits all necessary documentation to Programming, Training, and Quality Assurance Unit (PTQA) monthly.

Confers with supervisor to evaluate case priorities and participates in case consultations.

Performs other duties as assigned.

Supervisory Responsibilities

Has no direct supervisory responsibility for staff members.

Equipment Operated

Standard office equipment and office technology such as copy machines, personal computers and telephones.

Confidential Data

All court-related information regarding hearings, etc. that the employee may come in contact with by working in the facility.

Working Conditions

Potential exposure to hazardous situations that occur in the Court environment.  Must be able to tolerate exposure to evidence and testimony that may be disturbing, clients who may potentially be verbally or physically uncooperative, allergens, such as perfumes and dust.

Usual Physical Demands

While performing the duties of this job, the employee is required to sit, to speak clearly, and to hear. The employee must occasionally lift and/or move up to 10 pounds.

Knowledge, Qualifications, Skills, and Abilities

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.  The requirements listed below are representative of the knowledge, skill, and/or ability required. 

 

Demonstrated knowledge of adolescent growth and development, particularly as it relates to trauma, mental health and substance abuse; knowledge of family dynamics and human behavior; knowledge of best practices as they relate to this population; skilled in brokering community resources, identifying and utilizing appropriate resources; effective oral and written communication skills; skilled in case management, interviewing, maintaining good working relationships with a variety of groups; knowledgeable about the court process; ability to manage stress, time, confidential information while maintaining a professional demeanor; ability to flex hours and work outside of regular business hours to accommodate programs and potential late court hearings; reliable/consistent transportation.

Education and/or Experience

Bachelor’s Degree in Social Sciences, Education, or related field.

AND

Three (3) years’ experience working in the juvenile justice system and/or case management.

Licensure or Certification Requirements

 

Must possess a valid Ohio driver’s license and maintain valid automobile insurance, with a clean driving record and ready access to a safe functioning automobile.
